BOND INFO REQUEST PACKET
We appreciate the opportunity to assist you in obtaining surety bonds for your firm. To begin the bond application process, we ask that you download the appropriate documents and provide the following information:
- Bond Request Form
- Last 3 years CPA-prepared financial statements with A/R aging, open and closed job schedules.
- Most recent Internal Financial with A/R aging, open and closed job schedules (if available).
- Personal Financial Statement of Owner(s)
- Contractor Questionnaire
- Bank Reference Letter
- First 5 pages of most recently available tax return (corporate and personal and on any affiliated entities)
- Current certificate of insurance
- Copy of line of credit documents
- Copy of any trust involved
- Completed bond worksheet, if there is an immediate need
